In-Home Therapy (IHT)
Offering services to your family in your home allows us to connect and support your family in a safe and comfortable space. Our team may include both a Master level clinician and a Bachelor level paraprofessional who will take the time to get to know your family, identify your strengths, and understand what you need to move forward.
What to Expect
- A strength-based, comprehensive assessment.
- Our team will come to your home, or other space that you prefer, and meet with your family multiple times a week to get to know your family’s needs.
- Develop goals and ways you want to reach your goals.
- With your permission, we will connect with the school, and other providers, to fully understand what is right for your family.
- We are available 24/7 to support you and your family during emergency situations.

Therapeutic Mentoring (TM)
There are times when your child/youth needs to work on building skills. Some youth experience difficulties with communication, managing feelings, or other general life skills. Our one-on-one adult Therapeutic Mentors will connect with your child’s outpatient/clinical team and make a plan to practice the skills that are needed to be successful.
Our Therapeutic Mentors take the time to make a real connection with your child. The Therapeutic Mentors use your child’s strengths and preferences to teach, coach, and practice real life skills. Mentoring may take place in your home, school, community, or recreational setting, or wherever you and your child prefer.
What to Expect
- A Therapeutic Mentor who is focused and committed on building skills.
- Activity based and real-life experiences that practice skills.
- Weekly, planned meetings that focus on coaching, teaching, and practicing skills.
- Collaboration with your providers, schools, and supports (with your approval).

Community Service Agency
Community Service Agency (CSA)
Bay State Community Services is proud to be the Community Service Agency for the Coastal and Plymouth Areas.
The Community Service Agency is designed to “wrap around” your family and provide support, resources, and coordinate services that you and your family have identified.
There are two services available to you and your family
Intensive Care Coordination – Our Care Coordinators are specially trained to first look for your families’ strengths and, with you, identify your family’s needs. Together, we develop a Care Planning Team, which includes people whom you want to support your family: this may include schools, providers, family members, coaches, clergy or anyone you want to support your family’s goals.
Your family will lead the team in what will work best for you and your family, develop a plan, and will work to connect you with support and resources.
Family Partners-Family Support & Training – Our Family Partners, who are parents like you, have parented a child experiencing behavioral health symptoms. Family Partners provide you with support, resources, coaching, and real-life guidance. Family Partners help guide you through every step of the child service system.

Community Support Program
Community Support Program (CSP)
BSCS’ Community Support Program (CSP) offers community supports to individuals who have been experiencing behavioral health symptoms and or substance use disorders. We meet you in your community and together decide what would best help you. Supports may include transportation to and from medical appointments, coordinate your care with other providers, assist you in developing skills and help you to find the community resources that will best meet your needs.
We understand that coming out of a hospital or inpatient stay can be difficult. The BSCS CSP team can support you coming home by making sure you have all the services you need.

FIRST Steps Together
FIRST Steps Together
FIRST Steps Together program supports parents of children under 5 (regardless of where the child/ren live), and/or expecting parents who are experiencing substance use/abuse/misuse who are looking for recovery supports, parenting skills, and a supportive, accepting, and non-judgmental community of recovery.
The Family Recovery Support Specialist (FRSS) who will provide this service is someone in recovery who personally understands the challenges your family is facing. Our Recovery Specialists have completed the Recovery Coach Academy and have received specialized training in evidence-based parenting and recovery supports that are right for you. This program also has a clinician who is available to provide additional clinical supports – if you would like.
Participation in this program is voluntary and participants may engage in:
- Parenting Groups with other parents in recovery raising children
- Connections to Resources to assist with housing, benefits, childcare, and other needs
- Education on parenting approaches and skill development
- Advocacy and Support from staff who are in recovery
- Masters level Clinical Staff
- Care Coordination with other providers, including state agencies
- Flexible and responsive in-home or telehealth supports

Parents as Teachers (PAT)
We firmly believe that as a parent you know what is best for your child and family. The Parent as Teachers is a home visiting program that provides you with information on your child’s development and learning. Our certified parent educators will provide you with support, resources, and information on your child’s developmental stage and how to support healthy growth and development.

What to Expect
- Home Visits – Our Parent Educators will come to your home and share information on your child’s development and assess any concerns you may have.
- Developmental Assessment – Our Parent Educator will work with you to assess your child’s development, including developmental milestones, hearing, vision and language development.
- Community Connections – Our Parent Educators will provide opportunities to connect with other parent(s)/caregivers to share experiences and learn from one another.
- Resources – Our Parent Educator knows the community resources to support your clinical needs, financial planning, employment, medical and behavioral health needs. If needed, we will connect you to community resources for assistance with formula, diapers, etc.
Who We Serve
If you are a parent(s) or caretaker(s) of a child under 6 and live in Abington, Avon, Bridgewater, Brockton, Canton, East Bridgewater, Easton, Holdbrook, Rockland, Stoughton, West Bridgewater, or Whitman, we want to help.

Young Parent Program
If you are a young parent between the ages of 14-24 and are looking for non-judgmental and practical support with your educational, vocational, career, parenting and family support, we are here to help.
What to Expect
- Practical and hands on supports – Our team offers hands on, practical parenting and goal development to meet your needs as a new parent(s).
- Connection to resources – Every family has different needs. Our team works with you to identify your needs and develop a plan that is best for you and your family.
- Assistance with education, vocational, and career goals – Our team will take the time to fully understand your next steps with your education or work goals and provide supports and resources to reach those goals.

Massachusetts Youth Diversion Program
Bay State Community Services Youth Diversion team is committed to addressing the needs of youth who come to the attention of the legal system and increasing public safety. The goal of this service is to reduce the likelihood of future legal involvement by holding the youth responsible for their actions while supporting positive youth development.
What to Expect
Intake and Assessment – We will use evidence-based practices to assess and understand the youth and their parent(s)/caregiver(s) concerns and behaviors.
Develop Diversion Agreements – Together with the diversion coordinator, the youth will develop a diversion agreement that is designed to reduce behaviors. This plan will include understanding and identifying barriers and behavioral health needs.
Case Management and Support – The diversion coordinator will provide individual support and guidance to the youth, which includes family support.

Youth in Norfolk County who come to the attention of law enforcement, including school resource officers, court clerks, district attorneys, and judges who have the legal authority to divert a youth from a justice system under specific circumstances.
