This Team is available to individuals over 18 who need assistance navigating the behavioral health system and are experiencing significant behavioral health challenges or if you have complex medical needs.

Bay State Community Services is proud to be a partner with Riverside Community Care- Collaborative Network of Care.
The Behavioral Health Community Partner program is designed to provide you with coordinated care. Our team includes nurses, care coordinators, and individuals with lived experience who know about behavioral health resources that are available to you.
We assist you in navigating the entire system of care, including medical, behavioral health, social, and long-term supports and services.

What to Expect
- Together we will complete a comprehensive assessment of needs and services and create a personalized care plan.
- Your team will provide integrated and comprehensive care coordination which may include referrals to services and support.
- With your permission, we will work closely with primary care physicians, therapists, psychiatrists, nurse practitioners, registered nurses, visiting nurses, home aid helpers, and/or recovery coaches to ensure your needs are being met.
- Meetings with your care coordinator to assist you with organizing medical and behavioral health appointments, overcoming barriers, and understanding your concerns.
Funding
This no-cost program is for eligible MassHealth members referred by their providers or current case management agencies. Participation is voluntary.
